Terms and Conditions
I agree to all of the following terms and conditions set out by Clovelly Park Primary School OSHC in relation to fees, bookings and cancellations, etc as stated in the Family Handbook. I will also inform staff of any changes in personal information (address, contact details, etc) or information regarding any medical needs of my child/ children.
I will provide medications in original packaging, including dosage information and all medications provided and held by the service will be within the use by date and all medication documentation (plans) be current and updated when required.
Permissions
I give permission for my child/children to participate in supervised walking excursions within the local area.
I give permission for my child/children to participate in supervised excursions during Vacation care.
I give permission for my child/children to be photographed and for their photo to be displayed in circumstances deemed appropriate. (i.e Newsletters, displays)
I give permission for OSHC educators to apply sunblock to my child if required.
I will provide my child/children with a Sun Smart hat for outside play.
I give permission for my child/ children to watch G rated movies.
First Aid
I understand that if an ambulance is required for my child that I am responsible for any medical/ambulance costs incurred.
Bookings & Cancellation policy
All bookings must be made via “Fully Booked”. A minimum of 24 hours’ notice is required for cancellations.
Bookings cancelled with 48 hours’ notice will not incur a fee. Bookings cancelled with 24 hours’ notice or less will be charged for the session.
Vacation Care & Pupil Free Days
Once my child/ children are booked in to the Vacation care / Pupil free days via “Fully Booked” I give permission for my child/ children to participate in all programmed activities including incursions and excursions. A copy of all risk assessments are available to be viewed by parents. All bookings for Vacation care need to be completed two weeks prior to the commencement of the program to assist with correct staffing ratios and any transport arrangements.
Signing your child / children in or out – Before/ After School Care
It is the responsibility for parents to sign their child / children in to the service on arrival at the morning session and staff will sign them out to go to class. Staff will sign children into the service in the afternoon and parents must sign them out when they collect their child/ children. This is a legal requirement. [During the current Covid – 19 Pandemic staff will sign all children in and out on behalf of parents.]
Fees & Late Fees
I agree to pay the required fees for my child/ children’s attendances and accept the policies and rules of the service. If my child/ children are not collected from the service by the close of the session I will be charged a late fee of $2.00 per minute per child which will be added to my account. If staff are unable to contact a parent, they will contact an emergency contact.
Any account outstanding for more than two weeks will result in your account being suspended .Please note if your account remains unpaid your account may be referred to a debt collection agency to recover the amount owing.
